Passport Processing

The Polk City Community Library is a passport acceptance facility. Appointments are required for passport services. A separate appointment is required for each applicant. The link to schedule an appointment is at the bottom of this page.

  • If you are more than 10 minutes late for your scheduled appointment, it will be canceled.
  • Your appointment time includes time for a passport photo. You do not need to make a separate appointment for a photo.

What steps can we help you with?

  • We accept NEW passport applications, verify that the information is complete and correct, and forward the applications to the U.S. Department of State.
  • We can take passport photos. ($15)
  • We CANNOT accept passport renewal applications if the passport was issued less than 15 years ago.
    • Exception: Children must have an appointment and use form DS-11 even for renewals.

What do I need to bring with me? (click here for a full checklist)

  • Anyone applying for a passport, including children. 
    • Please note: Minors under the age of 16 MUST have both parents/guardians present or bring a completed and NOTARIZED Form DS-3053 (link below) from the absent parent/guardian along with a clear photocopy (no photographs) of the front and back of the same photo ID shown to the notary. 
  • Proof of Identity-example: Driver's License
  • Proof of Citizenship-example: State-Issued Birth Certificate or current or expired passport
    • Note: Hospital-Issued birth certificates are NOT acceptable. Rule of thumb: if it has a footprint on it, it is not acceptable.
  • Proof of any name changes-example: Marriage certificate, original adoption papers, etc.
    • If your driver's license has your married name on it, you do not need to present a marriage license.
  • Your application-You can fill this out and print it online, print it off and fill it out, or request a copy from us to fill out.
  • IMPORTANT: You must pay for your passport by check or money order. You will need a CHECK or MONEY ORDER payable to the U.S. Department of State. Payment for the library fees (passport execution fees and photos) may be made by cash, check, credit card (surcharge applies), or VENMO.
  • BEWARE of scam passport sites! Only use travel.state.gov. If a site directs you to pay online or offers you passport insurance, it is likely not valid.

How long does it take to get my passport?

  • As of December 18, 2023: Routine processing is 6-8 weeks and expedited processing is 2-3 weeks from the time your application arrives at the processing center.
