Passport for Minors Under the Age of 16

All children under age 16 must apply for a passport in person with two parents or guardians using Form DS-11. You cannot renew your child's passport using Form DS-82. Passports for children under age 16 are only valid for 5 years. 

Please note: Minors under the age of 16 MUST have both parents/guardians present or bring a completed and NOTORIZED Form DS-3053 (link below) from the absent parent/guardian along with a clear photocopy (no photographs) of the front and back of the same photo ID shown to the notary.

  • BEWARE of scam passport sites! Only use travel.state.gov. If a site directs you to pay online or offers you passport insurance, it is not valid.

A separate appointment is required for each applicant. Time is included for a passport photo if needed so no additional appointment needs to be scheduled.
