Sidewalk Repair Sign Up

The City of Polk City is partnering with Caliber Concrete of Adair to help residents complete needed sidewalk repairs throughout the community.

If you have identified sidewalk sections on your property that need repair and would like to participate in the City's coordinated repair program, please complete the form below. By joining the City's contractor program, residents may benefit from reduced project costs through group pricing and will be eligible for a waiver of the City's $75 sidewalk permit fee.

After submitting your information, City staff will coordinate with Caliber Concrete to review the requested repairs and provide additional details regarding the repair process, estimated costs, and scheduling.
